After enduring the heartbreaking loss of his 2-year-old daughter, Emily, to a medication mishap, Chris Jerry has committed his life mission to seek out technologies and methodologies that can forestall similar medication errors from afflicting others. Initiated in 2006, his relentless efforts have involved collaborations with top-tier medical technology innovators and global healthcare providers, unearthing healthcare strategies proven to be life-saving. His story underscores EQUASHIELD®'s commitment to tomorrow with zero medication errors utilizing lifesaving technologies.

The pharmacist who prepared Emily's dose, having served a half-year sentence in prison, completed 400 hours of community service and, endured a period of house arrest, agreed to a plea of no contest to involuntary manslaughter. Subsequently, in an extraordinary act of forgiveness, Christopher Jerry publicly pardoned Cropp. Their unique journey then took them across the country, visiting numerous medical conferences and facilities. They contributed to the medical community by providing Continuing Education (CE) and Continuing Medical Education (CME)-accredited lectures and presentations. These sessions offered their distinct insights into the tragic events that unfolded, emphasizing the importance of safety and efficiency in healthcare.

Michael R. Cohen

Michael R. Cohen, RPh, MS, ScD (hon.), DPS (hon.), is founder and president emeritus at the Institute for Safe Medication Practices (ISMP). He is active in many patient safety initiatives including co-editor of the ISMP consumer website, chairperson of the International Medication Safety Network, former member of the US FDA Drug Safety and Risk Management Advisory Committee (DSaRM) and the Nonprescription Drugs Advisory Committee (NDAC) and is currently a consultant to FDA. He serves as vice chair of the Nomenclature and Labeling Expert Committee for the United States Pharmacopeia.
Among the recognitions for his advocacy, understanding and contributions to medication safety, Cohen has received the John M. Eisenberg Patient Safety and Quality Award from the National Quality Forum and the Joint Commission, and the Harvey A. K. Whitney Award from the American Society of Health-System Pharmacists. In 2006, he was recognized as a MacArthur Fellow by the John D. and Catherine T. MacArthur Foundation. Cohen also has been awarded honorary Doctor of Science degrees by Thomas Jefferson University, the University of The Sciences in Philadelphia, and Long Island University. He also has a Doctor of Public Service from University of Maryland.
